The New York State Department of Health (NYSDOH) Bureau of Community Chronic Disease Prevention, Asthma Control Program seeks applications to establish and/or expand comprehensive asthma control services to improve health outcomes for people with asthma and reduce the burden of asthma in high-risk geographic areas of New York State (NYS). The aim of this request for applications (RFA) is to fund organizations serving high-risk areas to expand the availability and quality of evidence-based comprehensive asthma control services by improving the quality of guidelines-based health care, ensuring the provision of appropriate asthma self-management education, and promoting policies supportive of asthma control. Improved asthma control will be achieved through a population-based, sustainable systems approach designed to deliver seamless, guidelines-based comprehensive asthma control services across public health, community and health care sectors. The RFA seeks organizations that will work to reduce the racial/ethnic and community disparities in the burden of asthma, with a primary focus on children ages 0 to 17 years.

NarrativeProspective applicants are strongly encouraged to complete and submit a letter of interest (see Attachment 3). Prospective applicants may also use the letter of interest to receive notification when updates/modifications are posted; including responses to written questions. Letters of interest should be submitted via the Grants Gateway in the Pre-Submission Uploads section of the online application. A copy should also be emailed to asthma@health.ny.gov. Please ensure that the RFA number is noted in the subject line and submit by the date posted on the cover of the RFA. The letter of interest should include the proposed Service Area the applicant intends to serve. A list of organizations that have submitted a letter of interest and their proposed Service Area will be provided in the Questions and Answers document due to be posted on or around the date listed on the cover of the RFA. Submission of a letter of interest is not a requirement or obligation upon the applicant to submit an application in response to this RFA. Applications may be submitted without first having submitted a letter of interest.
