The Taconic Developmental Disabilities Regional Office (DDRO), Region 3 and the Taconic DDRO Family Support Services (FSS) Advisory Council are pleased to announce the availability of funding to provide supports and services to families that care for a family member with a developmental disability living at home. The goal of family support services is to assist families to remain intact for as long as they desire and avoid unnecessary out of home placement. The Taconic DDRO serves families and individuals in Dutchess, Putnam, Ulster, Columbia and Greene Counties whom are OPWDD eligible at the time of receiving services. Please refer to OPWDD Eligibility Definition (Attachment 3).
The Taconic DDRO is reinvesting FSS dollars in the amount of $206,884. Proposals are being accepted for Family Support Services that address priority needs. The priority needs that have been identified are:
1- Supports and services for those with behavioral and/or medical challenges that are unable to access existing programs,
2- Expansion of an existing program that provides quality programming/service and is able to demonstrate a documented unmet need,
3- Provision of transportation services and/or related supports to increase opportunity for access of and participation in qualifying programs and/or services, and,
4- Other innovative programs to meet a documented unmet need.
Note that names of individuals the proposed program/service will serve should be included in the proposal and there must be a documented demand for the proposed program/service in the identified geographic area.
Proposals must address one of the identified priority needs. Proposals should be for ongoing services and awards will result in the development of a multiyear contract for up to five years. Individual contracts may be less than five years to coincide with the calendar year or existing contract term.
Providers of Family Support Services under OPWDD must comply with all applicable NYS OPWDD regulations. Only incorporated not-for-profit agencies or other governmental agencies may apply. In order to participate in this RFP, an offeror needs to be an approved OPWDD provider.
In addition to Grants Gateway registration, all non-governmental Not for Profit (NFP) applicants must be Prequalified at the time and date that the application is due. In keeping with this requirement, any application from a non-governmental NFP which has a status other than one of the following at the time and date that the application is due shall not be considered for funding:
o Document Vault Prequalified;
o Document Vault Prequalified Open;
o Document Vault Prequalified/In review; or
o Document Vault Open for PQS Edits

Awards will be made for proposals addressing the identified local district priority needs as noted on page two, at the sole discretion of the FY 2017 RFP Review Committee. Anticipated program start-up for successful applicants will be April 1, 2018. Changes to this date must be reviewed/approved by the local DDRO. The grantee is responsible for communication with the DDRO in monitoring progress toward commencement of program. Continued annual funding is contingent upon satisfactory service performance and continued availability of state and federal funds.
