FAMILY SUPPORT SERVICES
FY 2017 RFP

The Developmental Disabilities Regional Office (DDRO), Region 1 Western and the WNY DDRO Family Support Services (FSS) Advisory Council are pleased to announce the availability of funding to provide supports and services to families that care for a family member with a developmental disability living at home. The goal of Family Support Services is to assist families to remain intact for as long as they desire and avoid unnecessary out of home placement.

The DDRO Region 1 Western office is reinvesting FSS dollars in the amount of $171,046. Proposals should be for ongoing services, and awards will result in the development of a multiyear contract for up to five years. Individual contracts may be less than 5 years to coincide with the calendar year or existing contract term.

The DDRO Region 1 Western office intends to fund one program, for the full $171,046. All proposals must be written for the full $171,046. Proposals requesting any amount less than the full amount will not be considered. Award will be made at the sole discretion of the FY 2017 FSS RFP Review Committee.

It is recognized that a variety of supports and services help families maintain their family members at home. Priority needs and have been identified, and are listed below. Proposals must address one of these priority needs:
• Family Reimbursement
• Behavior Support
• Expansion of an existing high quality Family Support Services program

Funding is available for families residing in Allegany, Cattaraugus, Chautauqua, Erie, Niagara, Genesee and Orleans counties. Individuals must have OPWDD eligibility and live at home with family at the time of receiving services. Proposals must address unserved/underserved individuals and their families with unmet needs.

Providers of Family Support Services under OPWDD must comply with all applicable NYS OPWDD regulations. Only incorporated not-for-profit agencies or other governmental agencies may apply.

Anticipated program start-up for the successful applicant will be 01/01/2018. Changes to this date must be reviewed/approved by the local DDRO. The grantee is responsible for communication with the DDRO to monitor progress towards commencement of program. Continued annual funding is contingent upon satisfactory service performance and continued availability of state funds.

In addition to Grants Gateway registration, all non-governmental Not for Profit (NFP) applicants must be Prequalified at the time and date that the application is due. In keeping with this requirement, any application from a non-governmental NFP which has a status other than one of the following at the time and date that the application is due shall not be considered for funding:
o Document Vault Prequalified;
o Document Vault Prequalified Open;
o Document Vault Prequalified/In review; or
o Document Vault Open for PQS Edits
